Collaboration And Proactive Teamwork Used to Reduce (CAPTURE) Falls: Gap Analysis Scorecard, Action Plan & Additional Tools

Purpose

The purpose of the CAPTURE Falls project was to implement the safe practice of inpatient fall risk reduction supported by a culture of safety, teamwork, and sensemaking. We trained interprofessional teams to collaborate and “make sense” of the risks associated with inpatient falls at three levels: (1) the patient, (2) the microsystem (unit), and (3) the organization. We implemented this intervention—Collaboration And Proactive Teamwork Used to Reduce (CAPTURE) Falls—in 17 Nebraska hospitals, 16 of which are Critical Access Hospitals (CAHs) with 25 or fewer beds. This project was supported by grant number R18HS021429 from the Agency for Healthcare Research and Quality.
