The Identification of Seniors at Risk (ISAR) tool has been developed for use in the emergency department. It represents one of the first steps in improving health care services to be offered to seniors to better meet their needs. Without the screening tool, seniors in the ED who are at increased risk for adverse outcomes could go undetected. The purpose of this guide is to introduce the ISAR screening tool in terms of content and methods of application in hospitals. Several healthcare professionals such as managers and other staff members collaborated with the research team in the development of this tool.
